The example gives each quarter 40% of the semester and the final 20%. Those are editable example weights, not a rule for every school.
Semester = quarter 1 × its share + quarter 2 × its share + final × its share
With quarters of 92% and 86%, the work graded so far averages 89%. It has earned 71.2 points toward the semester. A perfect final could bring the semester to 91.2%; a zero would leave 71.2%.

The table uses your selected cutoffs. It solves for the minimum final score that reaches each letter, using the exact cutoff. Rounding is a separate school rule.
For the example, reaching 90% takes a 94% final. Already secured means the numeric cutoff is reached even with a zero; school exam requirements still apply.
